clients
are required to fill in an intake form or review a previous form prior
to each session.
- The purpose
of this form is to alert the massage therapist of any new or existing
medical conditions and thus shape the massage session to provide
maximum benefit to the client. There are certain conditions where
massage therapy cannot be administered because it may further aggravate
a condition or cause harm. It is imperative therefore, that the
massage therapist be informed of all conditions and the vehicle for
this is the intake form.
- Client and massage therapist will have an opportunity to
discuss any issues or health concerns prior to the massage.
